Alliance Medical Center is currently hiring a full time Promotora/Community Health Worker:
The Promotora/Community Health Worker is responsible for reaching out to the community and assisting individuals and families to learn about important health information as well as help patient's access health insurance and other needed supports.
Requirements:
Essential Duties and Responsibilities include the following.
Conduct individual and other community outreach and educational sessions to inform current patients, stakeholders and other community members in health care topics of interest including staying safe from and dealing with Covid and other infectious diseases, the value of preventative cancer and other health screenings; keeps up-to-date on healthcare topics.
Schedule and orient new clients and potential clients to the Alliance Medical Center program(s) and notifies them of their rights and responsibilities as participants, using appropriate electronic portals as directed.
Complete applications for the following programs:
Medi-Cal, Covered California, County Medical, Access for Infants and Mothers Program, Gateway, Family Planning, CDP, and any other new upcoming healthcare programs.
Make appropriate referrals to agencies and facilities and works with clinic staff to see appropriate clients in individual programs.
Attend Covered California and Medi-Cal enrollment related meetings offsite or by teleconference.
Participate in outreach efforts in the community within the health center's Prevention and control of chronic disease Educate the patient and their families on the importance of lifestyle change; adherence to their medication regimes and recommended treatments Help patients navigate the healthcare system; enrollment eligibility, appointments, referrals helps figure out transportation, promoting continuity of care Assess how well a self- management plan is helping the patient meet their own health goals Other duties as assigned.
CHW has the following qualifications and skills:
To perform this job successfully, an individual must be interested in fostering community member's well-being, demonstrated by experiences and/or education regarding helping people.
Education and/or
Experience:
High school diploma or general education degree (GED); or one-year related experience and/or training with additional years of experience preferred but not required; or equivalent combination of education and experience; Community Health Worker certification preferred but not required.
Language Skills Must be fully bilingual and biliterate (Spanish/English).
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
Ability to write routine reports and correspondence, ability to speak effectively before groups of customers or employees of organization.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
